The hardest part of arranging support for a progressive condition is that what works now may not work in six months. Plans get written, services get set up, and then needs change — sometimes gradually, sometimes quickly.
Providers who can only deliver what was agreed at intake become a problem at exactly the point you need them most. We review regularly and adjust as things change, rather than waiting for a crisis or a plan review to catch up.
That might mean increasing hours, adding overnight or clinical support, changing how a task is done as mobility changes, or bringing in equipment. The aim is that you don’t have to renegotiate your care every time your situation shifts.
What we support with
Personal care — adapting technique and equipment as strength, mobility or fatigue change.
Nursing and clinical care — medication support, continence care, pressure area care and clinical needs delivered at home by registered nurses.
Domestic assistance and meal preparation — including support where swallowing or fatigue affects eating.
Transport and community access — keeping appointments and staying connected for as long as you want to.
Respite — planned and emergency, recognising that partners and family carers of people with progressive conditions carry a heavy and sustained load.
Overnight and 24‑hour support where it’s needed.
Working with your clinical team
People living with progressive conditions usually have a specialist team already — neurologists, MND or MS advisors, occupational therapists, speech pathologists, continence nurses. Good support work reinforces what that team has put in place.
We follow prescribed equipment use and therapy programs, and we’ll raise it early when something we’re seeing at home suggests a review is needed. Support workers are often the people who notice a change first, and that information is worth passing on.
We support NDIS participants, people with Home Care Package or Support at Home funding, TAC clients and private arrangements across Melbourne’s east, south‑east and the Mornington Peninsula.
